Palliative Care Community Services

The Swan Hill Palliative Care Community Service delivers specialist community palliative care to people residing within the Rural City of Swan Hill (excluding Robinvale), Shire of Gannawarra (excluding Cohuna) and the Shire of Buloke.

The Palliative Care Community Service provides care for the terminally ill which recognizes the need for physical, psychological, emotional and spiritual support for the patient, their family, friends and/or carers. The service works in a collaborative manner with local general practitioners, volunteers, district nurses, allied health and local government services to provide comprehensive care. Carers, family and friends are supported and encouraged in their care of the palliative care patient, which is delivered, where possible, in the environment of the patients choice.

The community service operates five days a week and access to a telephone after hours service can be arranged.

A bereavement follow up program is offered for 12 months with referral to appropriate health professionals for persons with complicated grief issues. The Palliative Care Staff and Volunteers hold an annual thanksgiving and remembrance service to enhance the bereavement program.
