Swan Hill District Health’s new Sub-Acute Service welcomed its first patient on the 23rd May 2019. The $2,860,000 project which was funded by DHHS and Swan Hill District Hospice Committee is located in the former first floor of the old Residential Aged Care Facility.

The 14 sub-acute bed facility includes 3 Rehabilitation beds, 3 Geriatric Evaluation and Management beds, 6 Transition Care Program beds, 2 Palliative Care beds and a therapy area.

The new facility is led by Nurse Unit Manager and staffed by a Geriatrician, Visiting Medical Officers, General Practitioner’s, 10 Registered Nurses, 7 Health Care Workers and supported by Allied Health Services (Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, Podiatry).
