Aged Care Services

Swan Hill District Health has two aged care facilities, Swan Hill District Nursing Home (46 beds) and Nyah Campus 'Jacaranda Lodge' (30 beds).

Our Aged care facilities are designed to be as Home Like as possible. Skilled nursing care is provided to meet the needs of the residents within a secure and caring environment.

Residents are supported in maintaining close links with the community, their family and friends as well as following their own interest wherever possible.

Planning and provision of care is a collaborative process involving the resident, family members, direct care staff, medical and allied health professionals and is individually tailored to suit each resident.

 

Jacaranda Lodge is located approx 27km’s north of Swan Hill in a rural setting.

Extended Care Unit is located in High Street, on the main campus site. The Extended Care Unit provides high care and respite care. If you require further information please contact the staff on 5033 9244 during office hours.
