Community Rehabilitation Centre

Services:

The Community Rehabilitation Centre provides short term multidisciplinary rehabilitation services to support those who have a disability, are frail, have a chronic illness or are recovering from injury/surgery to retain independence and transition back into the community.

Once a referral is received and triaged based on care needs, individuals are contacted for screening and/or comprehensive assessment to determine their care coordination and therapy needs. Our input can vary between individual and/or group therapies.

Specialist Clinics

A range of specialist clinics are offered at the centre that provide specialist assessment, diagnosis and management to support clients with specific conditions. Within these clinics, clients may be seen by specialist doctors, allied health professionals or nurses.

Please note, clinics only run on certain days, allocated times and locations. The service will endeavour to schedule appointments at your preferred time, however scheduling may be subject to availability. Please advise of cancellation or need to reschedule appointments at your earliest convenience to avoid delays in service provision.

Referrals:

GP referrals are preferred (and required for some specialist clinics); however anyone can make a referral to the service including health professionals, you or your carer. For self-referrals, a health summary from your GP should be included.

Waiting time:

All referrals to our services are assessed and prioritised according to your care needs and you will be contacted by the centre to advise if you have been waitlisted or allocated an appointment.

With your consent, referral acknowledgement and relevant correspondence will be sent to your GP to ensure continuity of care.

Information for referrers:

To improve access and appropriateness of referrals to public hospitals, the Department of Health and Human Services (DHHS) has developed state-wide referral criteria for certain medical and surgical conditions.

To allow efficient referral triage, please include the following essential information in your referrals. Please refer to: Minimum information for referrals to non-admitted specialist services

By having all relevant information, the triage teams will be able to determine the most appropriate service for your patient and arrange a timely appointment.

Information for clients:

Please come prepared on the day of your appointment. You can write down any questions you want to ask your clinician. Please bring your:

  • Medicare Card
  • Current medication list
  • Relevant test results or x-rays
  • Contact details of your GP 

We will try and see you as close to your appointment time as we can. Some patient appointments may take longer than others. This can lead to longer waits in clinics so please bring items you may need such as food or medication.

Pick-Up and Drop-off Zone

There is a drop off zone available, with entry accessible via McCrae Street.

Parking

There is some short term and long term street parking available in close proximity to the centre.  However, please note this is limited so please arrive early for your appointment to accommodate for parking.

Taxi

Please ask your taxi driver to drop you at the Community Rehabilitation Centre at Swan Hill District Health. They are encouraged to drop you off via our drop off zone. Reception can assist you to call a taxi on completion of your appointment.
