Services provided at the Allied Health area-

Podiatry- individual consultation regarding footcare, initial education and regular review appointments phone 5033 9390.

Services provided from 60 McCrae Street site (around the corner from main entry), Swan Hill

Dietetics- individual consultation regarding appropriate diet, initial education and review appointments phone 5033 9337. Supermarket tours are provided once per term, call us to find out more......

Diabetes Education- individual consultation regarding monitoring of blood glucose levels, insulin use, education on the disease and its affects, for appointments phone 5033 9337.

HEAL Program (Healthy Eating, Activity & Lifestyle):
This is an education program provided by a Dietitian and Exercise Physiologist over an 8 week period. It is relevant for those newly diagnosed with Diabetes or Pre-Diabetes and those who haven’t recently received any education. Phone 5033 9337 for further details.
Venue: Community Rehabilitation Centre, SHDH.

Diabetes Support Group- contact Anne on phone: 5032 3130 or Lyn on phone: 5032 2704, the group meets on the second Wednesday of even months, at Swan Hill District Health CRC meeting room from 7.30pm.
