Care planning program involving client, carer, clinicians, care coordinators, the clients GP and other medical specialist
for people suffering chronic diseases and complex needs. This includes people with Chronic heart failure and Chronic respiratory diseases.
The Program aims to reduce/eliminate avoidable hospital admissions.
