Swan Hill District Health’s new Sub-Acute Service welcomed its first patient on the 23rd May.The $2,860,000 project which was funded by DHHS and Swan Hill District Hospice Committee is located in the former first floor of the old Residential Aged Care Facility and will deliver 14 sub-acute beds and a therapy area that includes:
- 3 Geriatric Evaluation and Management beds
- 3 General Rehabilitation beds
- 2 Hospice/Palliative Care beds
- 6 Transition Care beds
- Therapy areas including Gymnasium
The new facility is led by Nurse Unit Manager, Melinda Hennessey and staffed by a Geriatrician, Visiting Medical Officers, General Practitioner’s, 10 Registered Nurses, 7 Health Care Workers and supported by Allied Health Services (Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, Podiatry).
