Diabetes Services

Many services are available to people with diabetes. These services provide education and support for people with diabetes and their carer's regarding diabetes self-management, lifestyle modification, equipment supply and support. 

Services provided at the Allied Health area 

Podiatry  - individual consultation regarding footcare, initial education and regular review appointments phone 5033 9390. 

Services provided from 60 McCrae Street site (around the corner from main entry), Swan Hill.....

Dietetics- individual consultation regarding appropriate diet for those with type 1, type 2, gestational diabetes and diabetes in pregnancy. Initial consultation and review appointments, phone 5033 9337. Supermarket tours are also available.         


 Credentialed Diabetes Educator Services

  • Diabetes education. Education and support for people with all types of diabetes: type 1, type 2, Gestational diabetes and diabetes in pregnancy. Individual consultation regarding monitoring of blood glucose levels, insulin use, education on the disease and its affects.
  • Continuous Glucose Monitoring This service provides professional continuous glucose monitoring over a 7 day period to assist in evaluating your diabetes management needs.
  • Endocrinologist access- via Royal Flying Doctor Telehealth Service. This is a bulk billed service.
  • Diabetes in Schools and Early Childhood Setting Seminars- This one day program offers staff in schools and early childhood settings the opportunity to expand their knowledge and become more confident in supporting and facilitating the day-to-day experience of young people with type 1 diabetes in their care. Sessions include information about diabetes and diabetes management, and practical workshops covering a variety of specialised topics in more detail including monitoring, pumps and camps. This training program has been developed by Diabetes Victoria, The Royal Children's Hospital Melbourne and Monash Children's Southern Health.

The diabetes service team, with your consent, liaises with your general practitioner (GP), Endocrinologist and other health professionals regarding your care.
