Quality & Risk

Our primary focus is of improving patient care. The unit is a resource to assist and support managers and clinicians in their patient safety and quality initiatives.

Functions include:

  • Quality improvement
  • Accreditation
  • Feedback, Compliments & Complaints management
  • Consumer participation
  • Policy & Protocols
  • Risk management

Swan Hill District Health values all feedback received from patients, clients, their relatives/carers and visitors. Feedback forms and boxes are located throughout the health service, as are posters explaining the procedure to provide feedback. All feedback is collected and managed by the Quality & Risk Department.

Feedback in the form of compliments, complaints and suggestions for improvement provide us with information about what we do well, and what we could do better. Information directing areas of improvement may also be captured in the various surveys conducted throughout the year, or via incident and hazard reporting. Whilst we endeavour to ensure that safe and quality healthcare is provided at all times, constructive feedback assists in the allocation of resources to make improvements in the most appropriate areas. On the other hand, compliments are an opportunity for consumers to acknowledge the wonderful work done by staff throughout our health service, while giving staff recognition for their efforts. Suggestions for improvement are taken seriously and all are considered independently with regard to relevance and feasibility. All feedback is discussed monthly at both the Continuous Quality Improvement and Risk Managers meetings, and appropriate actions taken.

Feedback boxes and forms are located in the Acute Ward, Midwifery Department, Emergency Department, front reception, CRC, Extended Care Unit, Jacaranda Lodge, Counselling & Health Promotion. Completed feedback forms can be placed into one of the feedback boxes, handed to one of our staff or posted to the Quality & Risk Department, Swan Hill District Health, PO Box 483, Swan Hill.
