Swan Hill District Health provides a range of Nurse Practitioner, Nursing & Allied Health Specialist Clinics. The Clinics provide specialist assessment, diagnosis and management support to clients with specific conditions. Within these clinics, clients may be seen by specialist doctors, allied health professionals, nurse practitioners or nurses.

Referrals

GP referrals are preferred (and required for some specialist clinics); however, anyone can make a referral to the service including health professionals, you or your carer. For self-referrals, a health summary from your GP should be included.

Appointment

When we receive your referral, it will be reviewed and categorised based on clinical urgency. While we attempt to provide you with the earliest possible appointment time, the most urgent patients are given priority.

You will be contacted regarding the outcome of your referral and whether further information is required, you have been placed on a waitlist and expected wait time, or allocated an appointment.

Please note, some clinics only run on certain days, allocated times and locations. The service will endeavour to schedule appointments at your preferred time, however scheduling may be subject to availability.

We do our best to run on time, however sometimes delays do occur.  Please be patient and if you have any concerns, please speak with our staff.   

Unable to attend your appointment?

If you are not able to make your scheduled appointment, please contact us as soon as possible so that we can make another time for you.  We can then advise someone else of an earlier time and this assists us to reduce our waiting times.

If you do not come to two appointments and do not tell us beforehand, you will not be given another appointment and you will need a new referral. 

What to bring to your appointment:

For your appointment bring:

  • Your appointment letter
  • Your Medicare card
  • A list of any medications you are currently taking
  • Any relevant test results or imaging

What happens after your appointment?

Your clinician will tell you if they need to see you again.

They will talk to you about what happens next and what you may need to do.

We share this information with your GP and other Health Professionals involved in your care, with your consent.

Need extra support or an interpreter?

Please let us know if you need extra supports due to having a disability or need cultural support.  

If you need the help of an interpreter, please let us know and we can provide a telephone/online interpreter for your appointment.
